There is no official form on which the Articles of Dissolution must be drafted or filed. The document must include the name of the corporation and the date the decision to dissolve was authorized. The text must also state that shareholders duly approved the dissolution in a manner consistent with state regulations and the entity’s Articles of Incorporation.
